Named after Dr. Perry A.
Klaassen, MD, who served for over 30 years as the
Medical Director for Community Health Centers, Inc.,
(CHCI) the Klaassen Family Medical
Center began operations
December 20, 2007.
The “PK Clinic” as it is commonly referred is
located in an area long overdue for community based
primary and preventive healthcare services.
CHCI and
Metro
Technology
Centers
partnered together on this endeavor to begin a
Communities In Schools Program. The mission of
Communities In Schools is to champion the connection
of needed community resources with schools to help
young people successfully learn, stay in school and
prepare for life.
The PK Clinic is located on the
Metro Tech Adult Continuing Education campus at
201 NE 48th Street
(48th & Stiles), just West of
Lincoln. Pediatric and Adult
healthcare, along with Behavioral Health and a host
of ancillary services are available. Appointments
are made through the CHCI call center. Call 769-3301
to make an appointment. We are not a walk-in
facility, appointments are encouraged.
Hours of Operation:
Monday - Thursday 8:00 a.m. until 4:30 p.m.
Friday 10:00 a.m. until 4:30 p.m.
Closed Daily for Lunch - Noon - 1:00 p.m.
